Each resident has an individualized and custom care plan that’s implemented by our skilled medical professionals. Each plan can be tailored to fit your particular needs, and we achieve this through a Personal Service Assessment. Our Clare Bridge Alzheimer’s and dementia care program was created over 25 years ago and continually embraces current research in dementia care, coupled with the experience of caring for thousands of people with dementia.
Our highly-trained staff takes this approach with every one of our residents. We are committed to having our memory care be centered on the person. Our program strives to make each person feel a sense of belonging and purpose.
We offer a gentle daily structure (seven days per week) choreographed with a minimum of six programs per day. All are designed to maintain the resident’s abilities and to encourage the use of their current skills.
