Brookdale Crown Point offers 38 residences and a comfortable, homelike environment which is ideal for individuals with memory loss. Our community is designed to reduce confusion, provide safety and allow complete freedom of movement in an effort to provide better and brighter days for each resident and their family members. Our residences are offered in private apartments and companion suites, which are perfect for providing an environment to develop new friends, provide companionship and offer cost savings.
At Brookdale, we are dedicated to maintain professional relationships with visiting physician groups, home health agencies and hospice agencies, as well as professional companion companies to support you and your loved one. These relationships allow residents to remain in their home here and often prevent the need to relocate to a nursing home.
Our community has tremendous pride and is fortunate to have longevity and tenure of its caregiving, nursing, dietary and housekeeping staff. Team members are committed to making a difference for the seniors we serve and take time to know each resident and their individual needs. At Crown Point, we are a family and the care we provide is truly exceptional.
Providing excellent dementia care is a complex task that requires ongoing guidance and teaching for our staff members. All associates receive initial and on-going training in proper techniques, approach and support for seniors with dementia.
This established set of protocols and techniques enables associates to intervene when challenging behaviors resulting from dementia arise. The whole-community philosophy is the key that empowers all of our staff to identify any unmet needs and be a part of the solution.
It is our goal to honor each person and their life stories by offering life enriching activities that complement their past traditions, hobbies and interests. Daily activities vary greatly, however, we are committed to fulfilling your loved ones spiritual, cultural, mental, physical and creative desires. We focus on providing engaging activities to meet the changing needs of our residents.
Our community believes in promoting an active lifestyle with a wide variety of activities each day, ensuring that your loved one is not only healthy but having fun. We believe in getting out into the greater community and are known to frequent local eateries and shops, as well as community events.
Brookdale is a pioneer in the development of memory care. We believe that no matter where an individual is in their life, each has the right to live in a dignified manner and enjoy meaningful experiences every day. Highly-trained associates deliver on this promise as they provide dementia care services which enable residents to experience daily moments of success.
Our community offers the Clare Bridge Alzheimer’s and dementia care program, which features peaceful surroundings and a setting designed to re-create environments people have enjoyed throughout their lives. Interior spaces are scaled and decorated like those of a private home, and enclosed walking areas give residents the opportunity to safely explore their surroundings.
The Clare Bridge Daily Path provides a gentle structure to each day and targets specific needs of those with Alzheimer’s and various types and levels of dementia. It features:
Morning Mental Workout which are daily late morning mental exercises including discussions, brain stimulating games and reminiscing.
Daily Physical Activity includes a variety of exercise opportunities including group exercises, walking programs and dancing.
Daily Life Skills are individual or group activities that provide a sense of purpose, meaning and belonging.
Person-Centered Life Enrichment Programming has activities designed using the residents own social history information as a foundation.
The Clare Bridge Dining Program was recognized as “Best of the Best” in the industry by Argentum, formerly the Assisted Living Federation of America (ALFA). This unique program was designed by a team of specialists to help those with Alzheimer’s and dementia master and enjoy successful dining experiences. It offers dementia-friendly menus, individualized dining assistance, a supportive environment and consistent seating arrangements.
Three savory home-style meals and three delicious snacks are served daily in our group dining area. Each meal offers a featured entrée and many alternate options are available to meet individual preferences. Delicious food, a friendly staff and great companionship make our dining experience special. We also provide easy-to-eat snacks between meals, helping to ensure that proper nutrition and calories are offered each day.
Designed by a team of culinary associates, experts specializing in dementia care and registered dieticians, our dining program sustains each resident, giving an opportunity to enjoy and master the dining experience. The program offers specialized aspects such as dementia-friendly menus, individualized dining assistance and preserving the use of utensils.
To help alleviate any confusion or added stress, we keep mealtimes very routine for our memory care community. All residents sit together at the same time and same table each day. Place settings are designed to enhance and provide ease for each person, and meals can quickly be converted to hand-held for those that prefer walking or standing, rather than sitting.
If you have a loved one in need of Alzheimer’s or dementia care, we cordially invite you to visit and experience our exceptional model and the programs we offer at Brookdale Crown Point. Please complete the online form or give us call to schedule a tour.
