Before a resident moves into their Brookdale home, our staff meets with their family to learn about their loved one’s life history, current cognitive abilities, daily routines, particular likes and dislikes, physical needs, and mental needs. Based on each resident’s needs and preferences, we design a Personalized Service Plan that can include the following physical assistance services:
In small “Cluster Groups” each resident is surrounded by others with the same cognitive abilities, to give them a feeling of fitting in. They participate in easy Daily Path activities that provide daily moments of success, and feelings of accomplishment and competence.
We select a minimum of six per day, tailored to match each resident’s previous life interests. These activities are scheduled to give our residents a gentle daily structure, seven days a week, in a routine that gives them a sense of security.
The Clare Bridge Dining Program was designed by a team of Alzheimer’s Care professionals, including a gerontologist specializing in dementia care, a chef, and a registered dietician. It offers:

Clare Bridge Crossings offers a new tier of care for seniors that bridges the gap between Assisted Living and Alzheimer’s and Dementia Care communities. For those with mild, early dementia symptoms whose needs fall in between the two traditional options of care, Crossings offers a new tailored level of assistance that some might call “Memory Care Lite.”
Specifically developed to aid seniors with early-stage dementia, the Crossings program designs activities that are tailored to each resident’s personal history, linking to your loved one’s past interests and involvements. Each activity is crafted to maintain your loved one’s abilities and enhance their social skills.
These Crossings activity options include vigorous mental exercises that employ the latest electronic technology at our big screen InTouch computerized station with the “It’s Never 2 Late” system. This easy touch-screen helps residents participate in “brain aerobics” that can include trivia games, quizzes, educational programs, memory workouts, and user-friendly Internet access.
Other activities like painting, woodworking and crafting also stimulate the creative thought process.
