Our community is dedicated to serving the special care needs of individuals with Alzheimer’s disease and other forms of dementia. From our building design to our carefully selected staff and specialized programming and management, we focus on the individual needs of each resident and family we serve.
Our community is conveniently located in the far North East Heights area of Albuquerque. Nestled at the base of the Sandia Mountains, we are convenient to a variety of shopping, banks and restaurants.
We are a quick 20-minute drive into downtown and to Albuquerque’s Historic Old Town with its wonderful shops, restaurants, museums and hotels lining the plaza. And, Santa Fe is just about an hour from our community. Should residents’ health needs require more intensive care, there is a skilled nursing home just down the street from our community.

We understand that every senior has a unique journey. We work to get to know every resident in both our assisted living and memory care communities to ensure they have the care they need to live a healthy, happy life.
Our campus provides memory care for those who have been diagnosed with Alzheimer’s or another form of dementia, our memory care community provides specialized care from a staff that is continuously trained in handling the unique needs of seniors with memory-related issues.
Residents with Alzheimer’s or related memory challenges reside in a supportive environment with familiar surroundings. By helping seniors achieve daily moments of achievement, residents find new meaning and purpose in their daily lives.
With a total of 31 private and shared studios, featuring beds, chest of drawers and a night stand. In addition, there’s either a private bath or one bath shared between two rooms. All of our studios offer views of the courtyard or mountains. Our memory care community is divided into two cottages, with each offering spacious living and dining areas.
Our floor plans allows for residents to move freely. The interior hallways are in a circular fashion, allowing residents to easily make their way around. Our secured courtyard is beautifully landscaped with pathways, flower beds, gazebos and benches. Everything is done for a reason but most especially to give residents a sense of freedom and independence, all within a safe environment over which our staff keeps careful watch.

Strong, Caring Professional Relationships
We nurture strong relationships with area hospitals, clinics, skilled nursing facilities, and rehabilitation centers. Through the communications and relationships that we share with key staff within these affiliations, we provide a seamless continuum of care for our residents.
We offer outside physical and occupational therapists as necessary to provide services in either the resident’s room or in one of our common areas. We also can provide hospice services as necessary with an option of three hospice agencies to choose from. Counselors also can be provided along with the services of a chaplain, should our family members need that type of support.
Our staff at Brookdale Juan Tabo Place is chosen for their caregiving skills and their caring attitudes. They are carefully selected and trained to manage the challenges associated with dementia, which can sometimes be difficult. The Clare Bridge Daily Path provides a gentle structure to each day that targets specific needs of those with Alzheimer’s and various types and levels of dementia. It features:
- Morning Mental Workout — Daily late morning mental exercises including discussions, brain stimulating games, and reminiscing.
- Daily Physical Activity — A variety of exercise opportunities including group exercises, walking programs and dancing.
- Daily Life Skills — Individual or group activities that provide a sense of purpose, meaning, and belonging.
- Person Centered Life Enrichment Programming — Activities are designed using the resident’s own social history information as a foundation.
Our staff is trained to understand and manage unpredictable behaviors and the need for an individualized approach for every resident, depending on the progression of the disease. We understand that creative problem solving is needed when providing dignity and quality of life to individuals with memory loss. For this we have dedicated dementia care specialists on staff to support our staff, residents and families.

Our staff, residents and family members are active in the community. This includes participation in the Alzheimer’s Association’s annual Walk to End Alzheimer’s to raise money for Alzheimer’s research.
