Qualified and nurturing caregivers provide services such as help with medication, bathing, dressing and coordinating appointments with outside providers. Our specialized care plans give an ideal blend of convenience and support with respect for individual desires.
We offer solutions for those in need of limited care and our customized approach focuses on being a trusted partner and listening to your needs. The lifestyle you want for yourself or your loved one is our priority, and we want to make certain that lifestyle is within reach.
It all begins with a personal service assessment. We take the time to listen to our residents so we can understand their needs, then we establish clinical, dining and program support that works for them in a meaningful way. Partnering with each resident, we recognize their individual preferences and adjust services to support them.
Personalized service plans are individually tailored and reviewed regularly by our staff to ensure we meet the changing needs of each person. We are available 24-hours a day and here to assist you with anything needed.
We value the privilege of knowing our residents and building relationships with them to create a truly personalized living experience. Whether having a unique snack, celebrating a favorite season or lifelong interest, we take the time to listen to our residents and know their desires.
Integral to life in our community are the programs, outings and opportunities for residents to engage physically, socially and with purpose. Our philosophy incorporates individual interests, as well as opportunities that allow them to make meaningful contributions to themselves and each other.
Our memory care offers resources, programs and a secure environment for those with Alzheimer’s disease or other types of dementia. We believe that no matter where an individual is in their life, each has the right to live in a dignified manner and enjoy meaningful experiences every day.
Deeply rooted in a person-centered approach, our memory care program focuses on sustaining feelings of belonging and purpose while seeking to preserve a sense of self. Providing excellent dementia care is a complex task that requires ongoing guidance and teaching for our staff members. All of our highly trained associates receive initial and on-going training in proper techniques, approach and care for seniors with dementia.
An established set of protocols and techniques enables our team to intervene when challenging behaviors resulting from dementia arise. Our whole-community philosophy is the key that empowers staff members to identify any unmet needs and be a part of the solution.
We are selective when choosing staff to join our team and serve our residents, and only those with a caring heart and experience working with seniors are accepted into our family.
Early-Stage Dementia Care
Persons in the early stages of Alzheimer’s disease and related dementias often experience loss of self-esteem and a lack of social engagement due to an increased awareness of their own cognitive decline. The programmatic concepts of the Clare Bridge Crossings program have as their foundation an increasing body of research that shows that a structured program of appropriate cognitive workout, regular physical exercise, and increased social interaction may help to slow the inevitable decline of Alzheimer’s disease and related dementias.
Clare Bridge Crossings is by nature a stepping-stone in Brookdale’s continuum of dementia care, and is not an appropriate setting for all persons with dementia. Managing the communities as a continuum is a critical piece of the success of Clare Bridge Crossings.
