We partner with each resident to create a personalized plan for both our Assistive Living and Memory Care (Alzheimer’s and dementia care) options. Each lifestyle can be tailored to a senior’s individual needs. Here’s a little about our two care options:
When you enter the front doors of our community, you can see why our residents love living here. Our Assisted Living community has 98 apartments, and each one is personalized to the resident’s liking. Some seniors cover the walls with family photos while others showcase their own art.
We offer a variety of apartment styles that boast many great features such as private bathrooms and kitchenettes. Our memory care neighborhood is a group of 30 studio apartments with a living room, country kitchen, community dining room and a beautiful and secure outdoor garden.
As a resident, you’ll get to know the faces of your neighbors and have the opportunity to build quality and long-lasting relationships. Our dining room is a place to enjoy a cup of coffee with friends before a meal or during one of our monthly family parties. The activity room is a place where a group of friends can get together for a card game or join one of our painting classes.
Our senior living community has an elevator for easy access to our second floor apartments and community areas like our library, television lounge and wellness center. You can head outside and enjoy our well-manicured grounds with a walking path and a porch complete with comfy chairs and a pond.
We value our resident’s opinions and feedback. Our life enrichment program is built around the specific interests of the men and women that live with us. The choices of social, physical, mental, spiritual, cultural and creative activities can be seen on our monthly calendar.
At our community there’s never a dull day. The calendar is packed with activities that enrich the body and soul. Exercise classes, hobby clubs and happy hours are just an example of what you might enjoy as a resident. We also take our seniors on outdoor excursions. We may get a sweet treat at our local ice cream spot, head into the Village of Brockport for shopping, or drive to Rochester for the Lilac Festival.
We understand that meals are an important of living at our community. For that reason, our three meals are offered restaurant-style each day. Daily choices of entrees are offered along with an “always available” menu so that your tastes can be met. Our dining services director will work with our resident care director to meet special dietary needs.
At our community, we can offer you or your loved one skilled Assistive Living or Memory Care. With each level of care, our staff is dedicated to supporting and caring for the individual’s needs. Whatever those may be.
Sometimes you may need a little extra assistance with daily tasks, such as bathing, getting dressed or getting to and from places. But the last thing you want is for a community to come in and control your world — ordering you around and dictating what you can and can’t do. Not the case with us.
We work with you, as a partner. Our aim is to provide you with the most amount of independence possible to live a safe and healthy lifestyle. We will sit down with you during a personal service assessment and listen to you. We don’t push a one-size-fits-all Assistive Living care plan on you. Instead, we will work together to create a custom and personal plan that you are comfortable with.
Our process is unique to Brookdale, and service fees are based only on what you need. We strive to deliver quality service and care to each individual, with regular review of service plans to meet residents’ changing needs. Our compassionate staff is trained to deliver assistance with respect for your independence. And they deliver that service with a smile.
Our approach to Memory Care is based on a few different factors. First of all, we incorporate the latest research and information on Alzheimer’s and dementia to provide our seniors with the best possible care. But we also factor in residents’ physical and behavioral needs, as well as life history.
We want to create a well-rounded care plan that utilizes the latest research while also providing an individualized and personal experience for our Memory Care residents.
In order to deliver this care in the right way, we carefully select our staff members to ensure that they will provide care with respect, compassion, and understanding toward our residents. Our staff members receive initial training, as well as on going training, on how to provide the proper care.
We value our residents’ well-being, and we create life-enriching opportunities for our seniors. Our goal is for residents to experience daily moments of success. We do this through the Clare Bridge Daily Path, which provides a gentle structure to each day that targets specific needs of those with Alzheimer’s and dementia.
Residents enjoy a minimum of six programs each day that are designed to maintain a resident’s abilities and encourage the use of their current skills. Programs may include a “Morning Mental Workout” where residents might enjoy brain-stimulating games or discussions, and a Person-Centered Program, which may honor a resident’s past interests and involvements.
