A Hospice Team Like No Other

Brookdale Hospice is made up of a team of compassionate and dedicated individuals who will be there to guide and support the patient and family. Our team is made up of:

Hospice Medical Director
The Hospice Medical Director is a member of the hospice team and supports palliation and management of the terminal illness and related conditions.

Registered Nurse Case Manager
The primary role of the RN Case Manager is to help manage pain and other symptoms and to teach caregivers how to care for their loved one. The nurse also ensures necessary medical supplies are ordered and assists in monitoring medications.

Chaplain
The chaplain offers pastoral care visits, as well as contacting a patient’s own clergy, pastoral counselor or others for additional support.

Social Worker
Facing a serious illness can be a time of tremendous pain and confusion. Services provided by the social worker include, but are not limited to:

  • Providing emotional support, counseling and guidance
  • Identifying available community resources
  • Assisting the patient and family with planning for funeral arrangements, additional care, nursing home placement, etc.
  • Offering information about advanced directives

Hospice Aide
The hospice aide assists with the patient’s personal care, which may include bathing, hair or skin care, shaving and catheter care.

Bereavement Services
The Bereavement Coordinator is available to assist families during this emotionally difficult time, as well as provide information on common aspects of grief. Bereavement support for grieving families is available for at least 12 months after the loss of a loved one.

Volunteers
Hospice volunteers are specially trained individuals who support the patient in a variety of ways, such as visiting, writing letters and more.



Levels of Care

Four levels of hospice care are covered by Medicare, Medicaid, and most insurance plans. These levels are listed below:

Routine Home Care
Members from the hospice team make intermittent home visits to ensure the patient’s symptoms are properly managed. The team also educates and communicates with the family with regard to the patient’s needs. This may include supplies, medications (relating to the patient’s terminal prognosis) and medical equipment.

General Inpatient Care
Some patients may have symptoms that cannot be managed at home to maintain pain control. Inpatient care is also provided in a designated center where the interdisciplinary staff can take an aggressive and individualized approach.

Continuous Care
Continuous care is provided at home during periods of crisis to help the patient manage pain and acute medical symptoms that are out of control.

Respite Care
This level of care is used to support the family and the patient. The patient may be temporarily admitted into an inpatient respite care center to give the caregiver a break.

Hospice care represents a full spectrum of help designed to respect the dignity of an individual facing a terminal illness.

At Brookdale Hospice, we focus on making people comfortable and giving them freedom from pain. Our compassionate associates partner with the individual to provide as much independence and control as possible for as long as possible.
Brookdale Hospice services are available on an intermittent basis, as needed, or after hours for urgent care needs. The services are covered under Medicare, Medicaid and many private insurance plans.

Brookdale Hospice can deliver care in a variety of settings, including a personal residence, independent or assisted living community or skilled nursing center. True to its caring nature, Brookdale Hospice is also sensitive to the patient’s cultural and religious beliefs, values and practices.
