The Clare Bridge concept was created over 30 years ago and has evolved by continually embracing the latest research in dementia care, coupled with the experience of caring for thousands of people with dementia and their families. Today, Brookdale is the leading provider of Alzheimer’s and dementia care in the United States.
Brookdale associates are trained in the concept of generating daily moments of success, and embrace this promise by offering opportunities for residents to find happiness and meaning by supporting their unique needs. The program is deeply rooted in a person-centered approach focused on creating feelings of belonging and purpose for our residents while seeking to preserve their identity and sense of self.
“These residents have people that care about them and people that love them. And I love them. Their lives still matter. That is what I do this for.”
– Elizabeth, Clare Bridge Program Coordinator
“I treat them as my mom or grandma.”
– Brooke, Clare Bridge Resident Care Associate
“I would not hesitate to recommend Brookdale to other friends and family in need of quality Alzheimer’s and dementia care. A special thanks to all of you who cared for my father, it was such a comfort to his family.”
– Susan & Helen Popp
moments of joy.
Over the last 30 years, Brookdale has cared for thousands of individuals living with dementia. We have the experience to understand the complexity of Alzheimer’s and dementia and what supports a person living with the disease requires. We’ve perfected evidence-based techniques and programs that address needs at all stages of the disease. From early to very advanced stages, our program supports well being every step of the way.
We place high importance on knowing the person and work to create a feeling of belonging and purpose for our residents. We understand what families coping with dementia are going through and have the experience to guide them through this journey.
We believe that no one should be defined by the symptoms of Alzheimer’s and dementia. Maintaining identity and a strong sense of self comes from living a life filled with meaning and purpose and contributions — we believe having dementia shouldn’t change that. Preservation of dignity and maintaining a person’s skills and interests is a critical element of living an optimum life.
Working in close partnership with the families of our residents is a hallmark of the Clare Bridge program. We understand that our families want to remain involved and a primary partner in their loved one’s care and daily path. We help our residents to live connected lives by nurturing the continuation of strong relationships with family, friends and their greater community. We listen and are here to support as we work together to create the best care environment and plan possible for our residents. Through our monthly education and support groups, families are provided with the opportunities to learn more about dementia and share their own journeys.
At Brookdale, we understand the nuances and intricacies of how the physical environment can impact and support those living with dementia, and our communities are built with that knowledge in mind.
Our Clare Bridge environments feature areas of the community that are set up for safe engagement in gardening, kitchen work, handy man help, and other pursuits that provide purposeful and meaningful successful moments. By honoring past interests and involvements, these programs tap into current skills and support successful experiences.
