Yes — we support patient-provider relationships, and many of our locations offer complimentary transportation services to and from appointments to facilitate continuity of care. We can help coordinate care so that it's simplified for residents, families and providers.
Brookdale proactively works with primary care physicians to help keep our residents healthy. Part of this involves maintaining electronic medical records through PointClickCare® (PCC) to keep track of immunization records, progress notes, vital signs, weights, medication orders, advance directives and diet orders. This can benefit you and your patient by having health information accessible in one place.
We offer a suite of services to help meet your patients' evolving care needs. We use collaborative care reviews to address our residents' changing clinical needs and lifestyle preferences. Additionally, we offer multiple service lines including independent living, assisted living, memory care, skilled nursing and private duty, as well as access to third-party auxiliary service providers such as outpatient therapy, home health and hospice.
