Memory loss doesn’t have to mean all is lost. If you and your family are looking for a memory care community, Brookdale Fort Collins can help. Our Alzheimer's and dementia care community, also known as Clare Bridge, is rooted in a person-centered approach. That means we help our residents find a sense of belonging and purpose while also helping them preserve who they are. We also have our Clare Bridge Crossings program which is designed for individuals in the early stages of Alzheimer’s or dementia. While not appropriate for all persons with dementia, this program is often a stepping stone between assisted living and our Clare Bridge program.
Where state-of-the-art care meets a passion for painting, residents stay engaged with a gentle daily structure of planned activities to help them maintain their abilities and encourage the use of their current skills. Brain games, dancing, cooking classes and music programs are just a few of the ways our residents enjoy their days. Along with our programming, you’ll have peace of mind knowing that trained staff is on-site 24 hours a day, seven days a week, to be alerted to an emergency.
Memory Care at Brookdale Fort Collins
We create personalized experiences for our memory care residents, spending time getting to know their pasts, likes and dislikes, and encouraging them to share stories and reminisce on their favorite memories to inspire joy and happiness. And with consistent care assignments whenever possible, we’re able to tailor care to each resident, helping to address their unique needs. Whether your family member needs support for early, middle or late stages of dementia, our memory care has purposeful programs that offer the care and attention you and your family need. Here are some of the programs and services we offer:
Clare Bridge is our exclusive brand of dementia care created more than 30 years ago.
Clare Bridge Crossings program is designed to meet the needs of seniors in the early stages of dementia.
Clare Bridge Solace is designed to meet the special needs of residents in the advanced stages of dementia.
ActivitiesOur residents have a lot of choices. Art classes, bingo and yoga are just a few of the activities included on our daily path of engagement. Your loved one can also enjoy devotional services, bowling, movie night and fun social events, tea socials and derby parties, too. We even plan off-campus regular outings for lunch, scenic drives and ice cream treats. Music is also an important part of life here with karaoke and sing-alongs.
DiningWe use mealtime as an opportunity to offer healthy menu choices and create a sense of belonging. Food is served on colorful plates and tablecloths to help your loved one more easily distinguish their food. And your family member will enjoy dementia-friendly menus with familiar selections that will appeal to their senses, small dining rooms with consistent pre-selected seating to help alleviate stress or confusion, and personal dining assistance as needed.
Our memory care program is deeply rooted in a person-centered approach. We offer the necessary resources and nurturing staff to care for those with Alzheimer’s and other forms of dementia.
