  1.  An individualized care plan designed around your patient’s unique needs. Through our personalized service assessment, we can identify the level of care and services your patient needs. Our assessment takes many things into consideration, including mobility, medications, dietary restrictions and diagnoses. From there, we create a plan built to help support your patient living with diabetes, supporting adherence to individualized care plans and helping patients safely manage their daily health needs. Care plans may include skin checks, pain management, support with diabetes monitoring and coordination with healthcare providers regarding laboratory testing and treatment plans, medication management, specialized diets, exercise plans, access to wound care and more.
  2. A proactive, whole-person approach, integrating lifestyle and wellness interventions alongside physician-directed care into our clinical care plans to help maintain or improve health. These may include exercise, nutrition assistance and spiritual wellness.   4. Routine health assessments and clinical interventions, which are essential for achieving positive health outcomes and helping reduce unnecessary hospital admissions and readmissions. Many diabetes complications are preventable or treatable if caught early enough. Routine monitoring allows us to track changes in a resident’s condition by comparing it to their clinical baseline. This allows us to promptly communicate meaningful changes and coordinate with you about appropriate interventions, such as dietary modifications or medication adjustments.
